HI Direct Primary Care
Enrollment now open for founding memberships

HI Direct Primary Care in partnership for your health.

HI Direct Primary Care is a membership-based practice offering prompt scheduling, thorough visits, and a provider who knows you.

A time-tested, proven model for you and doctor.

HI Direct Primary Care was built for a comprehensive experience. You'll have a provider who knows you, understands your health objectives, and is ready to walk along beside you on your healthspan journey, combining longevity and wellness.

Hawaii has a serious primary care shortage. For many residents, establishing care with a new provider may take months — and when the appointment finally happens, it can feel rushed. Direct Primary Care emerged in the 90's to preserve the sacred art of medicine.

Time to know you, your history and your goals.

Providers who take the time to actually know you.

HI Direct Primary Care is built on the foundation of some of the most experienced, deeply trained providers from Manakai O Malama — one of Hawaii's most respected integrative medical homes for over two decades.

Dr. Rachel Okabe

Rachel Okabe, MD

Internal Medicine, Lifestyle Medicine
HI DPC Assistant Medical Director, Board Certified Internal Medicine Primary Care

Dr. Okabe is a board-certified internal medicine physician and the clinical lead at HI Direct Primary Care. She graduated from NYU Grossman School of Medicine and completed her residency at NYU Langone Hospital in Brooklyn, where she also practiced primary care before returning to Hawaii.

Dr. Okabe is the co-author of 19 peer-reviewed publications in journals including Cancer Cell and Nature Medicine, and holds additional training as an Integrative Nutrition Health Coach. Patients consistently describe her as someone who listens deeply.

Dr. Ira Zunin

Ira Zunin, MD, MPH

HI DPC Medical Director
Integrative and Preventive Medicine

Dr. Zunin co-founded and led Manakai O Malama for nearly 25 years, caring for hundreds of thousands of families across the islands through a team-based, multidisciplinary model. A graduate of the UCLA School of Medicine with additional training in preventive medicine and public health, he curated a referral network of trusted specialists throughout his career. Dr. Zunin has stepped back from that role to offer the kind of in-depth, unhurried care that a smaller practice makes possible.

Body and Mind Research Institute

HI Direct Primary Care is affiliated with the Body and Mind Research Institute, LLC, an independent hub for clinical research. From time to time, members may be told about studies for which they might be eligible. Participation is entirely voluntary, plays no part in your care or your membership, and is always your choice.

What you need to know about HI Direct Primary Care.

We designed this practice so that most questions are answered before your first visit. If something isn't covered here, the best next step is to book your evaluation.

What is Direct Primary Care?
Direct Primary Care (DPC) is a membership model where you pay a flat monthly fee for access to a primary care physician — no per-visit billing, no insurance middleman for primary care services. Your membership covers routine visits, preventive care, Telehealth, direct and timely communication with your provider.
Is this a replacement for health insurance?
No. DPC is not a substitute for health insurance. You'll still need a traditional plan to cover hospitalizations, emergency care, surgeries, specialist visits, imaging, and other lab work. HI Direct Primary Care covers your primary care relationship — the ongoing, day-to-day connection with a provider who knows you.
How many in-person visits are included?
Your membership includes up to 6 comprehensive in-person visits per year as needed. Convenient Telehealth appointments and direct communication with your provider is included for prescription refills or other inquiries that may not require a full comprehensive visit.
What happens at the initial evaluation?
The initial evaluation is the first step to becoming a member of HIDPC. The evaluation is a one-on-one visit with your provider to discuss your health history, goals, and what you're looking for in a primary care relationship. Your provider can answer remaining questions you may have to determine whether HI Direct Primary Care is the right fit for you. If it is, the $250 evaluation fee is applied toward your membership.
Is there a fee to join, separate from the monthly rate?
Yes — a one-time Enrollment Fee of $150 applies when you join as a member, separate from your membership rate. If a membership is ever cancelled and later reinstated, the Enrollment Fee applies again at the time of re-enrollment.
How quickly can I get an appointment as a member?
Members can typically schedule in-person appointments within 2 to 3 business days. For non-urgent questions, you can reach your care team directly via MyChart messaging, email, or phone and expect a response within 24-48 hours.
Can I communicate with my provider between visits?
Yes. Direct communication with your care team — by direct messaging via MyChart, email or phone — is included in your membership. We're not a 24/7 on-call service, but you'll have easy access to your provider for non-emergency questions and follow-up between visits. Expect a response within 24-48 hours from your provider.
Do you see patients on the neighbor islands?
Yes. We typically recommend at least 1 initial in-office visit on Oahu to establish your care relationship, with Telehealth for ongoing follow-up. If you're on a neighbor island, mention that when you book and we'll discuss the options that work best for you.
Does my insurance cover DPC membership fees?
HI Direct Primary Care does not bill insurance for primary care services, and membership fees are not typically covered by insurance. Your insurance can still be used for prescriptions, diagnostic testing such as blood work and imaging, and referrals to specialists outside of this practice.
